You are probably scheduling the wrong thing

Most schedules are built around the practitioner, because the practitioner is who the patient is booking. Quite often the real constraint is something else: one treatment room, one machine, or the assistant who has to reset between appointments.

If two practitioners share one room, a diary that models two practitioners will cheerfully book two people into one space. The day then repairs itself by running late, and everyone blames the patient who arrived twenty minutes behind.

Time it on the floor for two weeks

The price list is a commercial document, not an operational one. A consultation listed at fifteen minutes routinely runs twenty-two once the conversation, the notes and the changeover are counted.

Book on the listed figure and the day is oversold by roughly a third before it starts. There is no scheduling system that recovers from that, because the problem is arithmetic rather than software.

Leave room on purpose

A schedule at 100% utilisation has no capacity to absorb a single overrun. The first delay of the morning propagates through every appointment after it and grows.

Deliberate gaps feel like lost revenue and are cheaper than the alternative, which is a waiting room full of people who will mention the wait to everyone they know.

Make cancelling easy, which sounds wrong

A meaningful share of no-shows are people who intended to cancel and found it awkward. They did not want to ring during clinic hours and explain themselves to a receptionist, so they simply did not turn up.

Give them a one-tap cancellation and a proportion of those become slots you know about with several hours' notice, which is enough to fill from a waiting list. Practices resist this because it feels like inviting cancellations. In practice it converts silent losses into recoverable ones.
