Worked scenario

Lenses ground to a number nobody verified

A dispensing error caught at collection costs the lenses, the remake and the customer's confidence, and it usually started as a transcription.

3 min read

A prescription is a small set of numbers where a single digit changes the product entirely. It is transcribed from paper into an order and then travels to a lab.

Verification before the lab, not after

Every check after dispatch is a remake. A second look before the order goes costs seconds and is the only point where the error is still free.

Cause is the useful record

Remakes recorded as remakes tell you nothing. Split into transcription, lab error, fitting and customer adaptation, the pattern points at a specific fix, and transcription is usually the largest.

What changes

  • Prescription entered once and checked against the source
  • Entry verified before the order goes to the lab
  • Remakes recorded with a cause, including transcription
  • Practitioner sources with frequent ambiguity identified
