A rejection you cannot trace to your own inspection record is a rejection you will concede, whether or not you were at fault.

A customer returns parts as out of specification. The manufacturer inspected before dispatch and believes the parts were within tolerance.

Belief is not evidence

Where inspection records are filed by date rather than tied to a batch, retrieving the relevant one takes long enough that most manufacturers simply accept the rejection and replace.

Aggregation finds the real cause

Rejections handled one at a time look like isolated incidents. Recorded against batches and causes, they usually concentrate on a machine, a shift or a particular dimension — which is fixable, unlike bad luck.
